Client Journey for Health & Wellness Services
Step 1. Initial Contact and Intake
Every journey begins with a warm and respectful connection. Whether referred by a healthcare provider, community partner, or you found us on your own, everyone is welcomed by the Open Door Group team. The intake process ensures a smooth transition into our services and offers an opportunity to learn about your background, strengths, challenges, and goals. You will also hear about the range of support available through our integrated health and wellness and employment services.
Step 2. Assessment and Wellness Goal Setting
Following intake, you will engage in a comprehensive assessment led by a trained staff member. This assessment explores areas such as physical and mental health, housing stability, daily routines, social supports, interests, and employment readiness. It also considers your personal vision for the future.
From this assessment, you and a facilitator co-create a set of wellness goals. These goals are rooted in your own definition of success—whether that includes improving mental health, reducing isolation, rebuilding confidence, gaining life skills, or preparing for employment. Goals may be short-term or long-term and are flexible to evolve as you progress.
Step 3. Personalized Wellness Planning
Once goals are defined, a personalized wellness plan is developed. This plan outlines specific services and supports that will help you move toward their goals. This may include:
- Participation in group programs such as horticulture therapy, arts and recreation, mindfulness, or peer-led wellness workshops
- One-on-one coaching focused on life skills, self-care, emotional regulation, or navigating health systems
- Social connection opportunities through supported community activities and volunteer placements
The plan is led by you, so you decide the pace, what’s most important, and how involved you want to be based on what feels right for you.
Step 4. Program Participation and Ongoing Coaching
You will be supported as you begin actively engaging in wellness programs and 1:1 coaching. These services provide both structure and flexibility, allowing you to explore your interests, build daily routines, and develop new coping strategies.
Regular check-ins with you ensure progress is reviewed, successes are celebrated, and plans are adjusted as needed. Coaching continues to play a vital role in identifying strengths, building confidence, and addressing any barriers as they arise.
Step 5. Supported Transitions
As you grow in your wellness journey, you may begin to explore new opportunities. Through Supported Transitions, you can access additional pathways tailored to your evolving goals:
- Supported Employment Services – including job exploration, skills training, job search, and individualized support in the workplace
- Supported Education Services – including access to adult education, tutoring supports, and help navigating school systems
- Supported Leisure Services – maintaining engagement in wellness and recreation activities that provide joy and balance.
You can move between or combine services, depending on your needs—there is no single path or timeline.
Step 6. Sustainable Independence and Community Involvement
The final stage of the journey focuses on building long-term independence and meaningful community involvement. For some, this means stable employment or returning to education. For others, it may mean increased self-confidence, stronger relationships, and regular participation in community life.
Those who have completed the wellness path continue to have access to drop-in programs and check-ins as needed, ensuring they feel supported even after reaching their goals. Success is self-defined, and we are here to walk alongside each individual as they create a fulfilling and connected life.
